How do I notarize an affidavit?

With Proof, an affidavit will be generated for you after the completion of serve attempts on a job. Our Affidavit Team will upload the generated affidavit to the job, and you will receive a notification letting you know that it is available to be notarized.

Affidavit Notifications

Proof does pay for printing at over 1500 FedEx office locations nationwide. To print an affidavit to be notarized, select the job in your Proof mobile app, then in the top right of the overview screen, select the drop-down menu and select “Print Affidavit at FedEx”. You will then be able to choose your preferred FedEx location and your documents will be sent and made available for pickup.

If you choose to print the affidavit yourself, you can do so by downloading it from your Proof mobile app to your phone, or by downloading the affidavit from your email to your laptop or desktop computer and following your normal printing procedures. Please note, Proof does not reimburse for printing documents yourself.

Affidavit Print Options

Once notarized, you can upload the affidavit directly to the appropriate job. Here's how:


Option 1: Download and Upload from Mobile

  1. Select the job in your Proof mobile app, and open the drop-down menu in the top right corner. You can choose to print the affidavit at FedEx via the app, or, you can download the affidavit to your mobile device.

  2. Once the affidavit has been notarized, clearly scan and save it as a PDF document to your mobile device.

  3. Select the job again in your Proof mobile app. Under the server name, click on the “Upload Affidavit” button. Select your saved PDF and submit.

Option 2: Download and Upload from Computer

  1. Open the email from Proof that contains the affidavit. This file will be a PDF document, which you can download to your computer or mobile device.

  2. There is a link in that email - please click this link, and it will bring you to the correct page (on the Proof website) to upload the affidavit as a PDF file once it has been notarized.

    ***If you are doing this on your computer, you’ll need to save the .pdf file of the notarized affidavit to your computer first.

    ***If the email link is unavailable or does not work, you can manually (on a web browser) go to www.proofserve.com, sign in to your account, look under “My Jobs” and click on the job you want to upload the affidavit for.

  3. At the top of the job page, you will see a button that says “UPLOAD NOTARIZED AFFIDAVIT". Click that button and you’ll be able to browse your files. Select the correct file and upload the notarized affidavit (PDF format).
