With Proof, affidavits are automatically generated for you after the completion of serve attempts on a job. Once the affidavit is ready, our Affidavit Team will upload it to the job, and you will receive a notification informing you that the document is available for notarization.

Use the In-App Scanner to capture and upload your signed affidavit directly from the job. It’s faster and more reliable than third-party scanning apps, and when you scan in-app you don’t need to upload exhibit photos—Proof reconstructs the affidavit using your full-color attempt photos already on the job.
Note: If you upload a PDF instead of scanning in-app, you must still include exhibit photos.

You will be notified via the Proof mobile app once your affidavit has been uploaded and is ready to be notarized. Be sure to enable notifications to stay updated on the status of your jobs.

🖨️ Printing Your Affidavit
Print at FedEx Office
Proof covers the cost of printing affidavits at over 1,500 FedEx Print & Ship locations nationwide. Here’s how to print your affidavit at a FedEx Office:
Open the Proof mobile app and select the relevant job.
Tap the drop-down menu in the top right corner of the Overview screen.
Select Print Affidavit at FedEx Office.
Choose your preferred FedEx location.
Your affidavit will be sent to the selected FedEx location and made available for pickup.
Print Independently
If you prefer, you can print the affidavit yourself:
Download the affidavit directly from the Proof mobile app to your phone.
Alternatively, download it from the notification email to your laptop or desktop and print it using your regular printing method.
Please note: Proof does not reimburse printing costs if you choose to print the affidavit independently.

📄 Uploading a Notarized Affidavit
After notarization, you can easily upload the completed affidavit back to the job. Here's how:
🧾 Option A (Recommended): Scan and upload in the Proof app
Open the job that needs a Wet Ink affidavit.
Tap Upload Affidavit at the bottom of the job.
Choose Scan Document and follow the prompts to scan each page (the required page count shows at the top).
The app will display the required number of pages at the top of the screen.
Use the scanner tools to manipulate your document. There are options for:
Filters - used to correct lighting/visibility
Flatten (recommended) - used to make any bent or folded documents appear flat
Cropping Tool - used to remove and backgrounds or photo edges
Rotate Image - used to flip images to the correct orientation
Review the pages and Submit. We’ll rebuild the affidavit automatically with your attempt photos—no exhibit uploads needed.
📌 Important: The exhibit-photo skip only applies when using the In-App Scanner. If you choose to upload a PDF, you must include exhibit photos.

📲 Option 1: Download and Upload from Mobile
Select the job in your Proof mobile app, and open the drop-down menu in the top right corner. You can choose to print the affidavit at FedEx via the app, or, you can download the affidavit to your mobile device.
Once the affidavit has been notarized, clearly scan and save it as a PDF document to your mobile device.
Select the job again in your Proof mobile app. Under the server name, click on the Upload Affidavit button. Select your saved PDF and submit.

🖥️ Option 2: Download and Upload from Computer
Open the email from Proof that contains the affidavit. This file will be a PDF document, which you can download to your computer or mobile device.
There is a link in that email - please click this link, and it will bring you to the correct page (on the Proof website) to upload the affidavit as a PDF file once it has been notarized.
💡 If you are doing this on your computer, you’ll need to save the .pdf file of the notarized affidavit to your computer first.
💻 If the email link is unavailable or does not work, you can manually (on a web browser) go to www.proofserve.com, sign in to your account, look under My Jobs and click on the job you want to upload the affidavit for.
At the top of the job page, you will see a button that says UPLOAD NOTARIZED AFFIDAVIT. Click that button, and you’ll be able to browse your files. Select the correct file and upload the notarized affidavit (PDF format).

Scan in-app (Recommended): Fastest, highest quality, and no exhibit uploads needed.
Upload a PDF: Use only if you can’t scan in-app. Exhibit photos are required.
