Understanding the Proof Cover Sheet

Learn what the Cover Sheet is, what information it contains, and how to use it to quickly access and prepare for service.

Want all the important job information in one place? The Proof Cover Sheet is your starting point and refresher guide for every service you claim. It provides a clear overview of everything you need before making a service attempt, including:

  • Job Details

  • Service Expectations

  • Document Information

  • State-Specific Service Rules

  • Client Specifications (that may apply)


Every job includes a Cover Sheet that prints along with the service documents. You may print your documents for free at FedEx, or choose to print them on your own. Please note that personal printing costs are not reimbursed.


Each cover sheet also contains a QR code in the upper-right corner for quick access to the job in the Proof app. Simply scan the QR code with your phone's camera to open the correct job directly in the app.

This helps eliminate the need to manually search for jobs and lets you get started faster. Please use the video below for reference:


At the top of the cover sheet, you will also find the servee/entity name prominently displayed, along with the job number and service address. The cover sheet also highlights important deadlines and service details, including:

  • Service speed/type

  • First attempt due date

  • Final completion due date

  • Registered Agent (if the serve is a business/entity)

  • Physical description of the servee (when provided by the client)

  • Additional Notes or Client Specifications (when provided by the client)

Further down the page, you'll find the Documents List, which identifies all documents included in the service packet. Any special instructions provided by the client will appear in the Client Specifications section. Always review and follow these instructions/expectations before attempting service.

Important Reminders:

  • Please review the entire cover sheet before your first attempt to ensure you understand all requirements.

  • Proof cover sheets are for your reference only and should never be served with the legal documents.

  • If you are instructed to serve or post documents in an envelope, ensure the envelope is plain and free of any company names, logos, branding, or identifying marks.
