How do I certify my affidavit?

With Proof, an affidavit is automatically generated for you after the completion of serve attempts on a job. Once the affidavit is ready, our Affidavit Team will upload it to the corresponding job, and you will receive a notification indicating that it is available for certification.

You can conveniently review and e-sign the affidavit directly within the Proof app!

Receiving Affidavit Notifications

You’ll be notified via email and through the Proof mobile app when your affidavit is ready to certify. Be sure to keep your notifications enabled to stay updated.

How to Certify an Affidavit

Certifying your affidavit in-app is simple. Here’s how:

  1. Open the notification email and click on the certification link — or

  2. Open the Proof mobile app, navigate to the specific job, and select Certify Affidavit from the drop-down menu.

Reviewing the Affidavit

Before certifying:

  • Carefully review the affidavit details. You’ll be able to preview the document before signing.

  • Please note: The information entered on the job’s Details page will automatically populate into the bracketed sections of the affidavit once it’s signed.

Take a moment to confirm that all information is accurate before proceeding.

Certifying and Signing

Once you're ready:

  1. Check the confirmation box to acknowledge the information is correct.

  2. Slide the toggle button to certify and e-sign the affidavit.

After certification, a copy of the signed affidavit will be automatically emailed to you for your records.
