My affidavit needs a change or amendment. How do I request it?

There may be times when an affidavit needs to be changed after a serve is completed. From the new Proof design, there are two main ways to request an affidavit edit if necessary.



Request through the Serve Tracker


Once the serve is complete and the health status on the serve tracker is showing that the serve is complete, you'll have two Quick Actions you can select:

  1. Download affidavit

  2. Report an affidavit issue


By selecting Report an affidavit issue, this will flag an affidavit for issues, and we will display a new label next to the affidavit that states, "Proof is correcting your affidavit".

While this is under review, it may look like this under the Serve Tracker:


​If different process servers provide multiple affidavits, you can see which one might be worked on via the Affidavits tab:



Request through the Flag feature


While you are on the specific Serve page, in the top right corner, you will see options to "flag". It will look like this:


If you click on Flag, it will allow you to flag your serve for multiple reasons. For affidavit edits, you can select Affidavit Changes as the reason.


There will be a Reason Detail drop-down to select the appropriate details for requesting affidavit changes, along with a description field that provides context and details for Proof to review. The information provided in the Description box is what Proof will use to update the affidavit.

Once submitted, the affidavit will be under review for those changes.
