After submitting the job, you will be presented with a pop-up box where you can label your documents as you would like them to appear on the affidavit.
Just click "Customize Document Names Listed on the Affidavit." Upon proceeding, you will be directed to a screen where you may input the desired document labels.
Once you have added the desired labels, click on "Save." You will then be directed back to the pop-up box.
* Please note that how these documents are named in this field is how they will appear on the Affidavit of Service.
If you cannot provide additional details, you may select the option labeled "I'll Finish Later" and revisit the task through your account portal. However, if you "β" out of the box, you cannot retrieve it.
